Pay: $2,546.40 weekly

Shifts: M-F 8am-5pm; On-call required on rotating weekends

This role requires providing comprehensive assessments and care coordination for high-risk members, within an onsite capacity. Key Responsibilities of the RN - Case Management - Travel
  • Perform comprehensive assessment of high-risk members
  • Collaborate with primary care providers to implement and engage comprehensive care plans
  • Continuously reassess and address cost-efficient, appropriate levels of care
  • Facilitate non-medical support to ensure compliance with treatment plans
  • Coordinate care transitions as needed
  • Work with physicians, social workers, and claims professionals in care transitions
  • Engage specialty resources as required for optimal resolution
  • Maintain detailed records of clinical, functional, and fiscal outcomes
  • Identify opportunities for health promotion and illness prevention
  • Act as patient advocate by protecting privacy and confidentiality issues

Requirements of the RN - Case Management - Travel
  • Associates or Bachelor's Degree in Nursing or related field
  • Current, unrestricted state Registered Nurse license
  • Ability to analyze complex medical information and make rational decisions
  • RN Licenses in IN, compact license accepted
