Key Responsibilities of the RN - Case Management - Travel

Perform comprehensive assessment of high-risk members
Collaborate with primary care providers to implement individualized care plans
Address and reassess cost-efficient, appropriate levels of care
Coordinate care transitions as needed
Engage specialty resources to achieve optimal resolution
Keep detailed records of clinical, functional, and fiscal outcomes
Identify opportunities for health promotion and illness prevention
Act as patient advocate by protecting privacy and confidentiality issues

Requirements of the RN - Case Management - Travel

RN License for WI
Hospice and wound care experience
Experience with Home Care Home Base (HCHB) highly desired
Certified CPR/BLS
Valid driver's license and reliable vehicle with valid car insurance
Completed skills competency checklist
Training in elder abuse, HIPAA, infection prevention, and OSHA
Case Management certification
Associates or Bachelor's Degree in Nursing
