Key Responsibilities of the RN - Case Management - Travel- Perform comprehensive assessments of high-risk members
- Collaborate with primary care providers to implement comprehensive care plans
- Address and reassess cost-efficient, appropriate levels of care
- Ensure non-medical support is in place for treatment plan compliance
- Coordinate care transitions and engage specialty resources as needed
- Maintain detailed records of clinical, functional, and fiscal outcomes
- Identify opportunities for health promotion and illness prevention
- Act as a patient advocate, protecting privacy and confidentiality
Requirements of the RN - Case Management - Travel- Associates or Bachelor's Degree in Nursing or related field
- Current, unrestricted state Registered Nurse license
- Ability to analyze complex medical information and make rational decisions
- Case Management certification
- Experience with HCHB, OASIS, and home health
