Travel Nurse RN - Case Manager - $2,550 per week in San Jose, CA

Job Title: Case Manager Profession: Manager Specialty: Case Duration: 13 months Shift: M-F Hours per Shift: 8:00 AM - 4:30 PM Experience: Minimum of two years of nursing experience in a clinical setting with a frail or elderly population License: Unencumbered RN License Certifications: BLS Must-Have: - Nursing knowledge and skills necessary to treat frail, elderly participants and manage complex clinical situations - Excellent clinical, organizational, and communication skills in settings with seniors, their families, and interdisciplinary team members - Ability to work independently with minimal supervision Description: The Case Manager is a member of the Interdisciplinary Care Team. The role involves working in collaboration with the team to deliver and manage all aspects of participant care. The Case Manager will conduct required assessments and develop and follow individualized participant care plans. They will address patient needs, communicate with participants and their families, and advocate for participant needs. In partnership with the Primary Care Provider, the Case Manager is responsible for delivering care in various settings including participant homes and skilled nursing facilities. They will manage participant and family care needs. Duties include taking ownership of the outcomes and quality of care for their assigned participant panel. This involves working directly and in tandem with the Primary Care Provider and utilizing clinic resources effectively. They will conduct required assessments every six months and educate participants on their conditions. Urgent and routine follow-up visits will be performed as determined by the participant’s needs. The Case Manager will develop and implement a discipline-specific plan of care in collaboration with the interdisciplinary team, participants, and families. They will identify problems and needs of participants and escalate them appropriately. Coordination of care for participants with a high level of complexity will be performed to ensure seamless transitions of care and participant satisfaction. They will manage all communications relevant to participant care needs. Additional responsibilities include documenting ordered procedures and care, communicating changes in participant status to the team, and reviewing test results. The Case Manager will ensure support on specific tasks as needed to enhance clinic operations. This role requires proficiency in handling multiple tasks and effectively prioritizing in a fast-paced environment. Demonstrated ability to work with diverse backgrounds and cultures is essential.
