Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8-hours (08:30 - 17:00) Experience: 2 years of experience required License: Active RN license in the relevant state Certifications: Relevant certifications preferred Must-Have: Acute hospital experience required Epic experience required Float required to any inpatient unit Weekend coverage required: one weekend per month Description: The position involves working as a Case Manager. This role operates Monday through Friday from 08:30 to 17:00. There are no call requirements associated with this position. The Case Manager will float to any inpatient unit as necessary. If scheduled to work on a weekend, the candidate will work a weekend day in place of a weekday shift. Acute hospital experience is essential for this role. Experience with the Epic charting system is mandatory. Candidates must have two years of relevant experience, but first-time travelers will be considered. Local candidates are accepted without any change to the bill rate. Additional skills preferred include acute hospital knowledge, admission criteria understanding, appeals and denials navigation, care coordination expertise, and familiarity with CMS guidelines. Knowledge of discharge planning, HIPAA guidelines, needs assessment, and prior authorizations is also beneficial. The candidate will interact with interdisciplinary support teams including IV teams, physical therapy, respiratory services, social services, and pharmacy among others. Experience with various patient age groups, including neonates, adolescents, adults, and geriatrics, is essential. The position requires proficiency in determining medical necessity per evidence-based guidelines. Candidates should be prepared to manage patient care effectively, ensuring the best outcomes for inpatient acute care patients.
