Acute Care Case Manager
Location: Torrance, CA
Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (Day); every-other-weekend rotation required and must be available for holidays
Hours per Week: 40
Candidate Pay Rate: $2600- $2850 per week

Roles & Responsibilities
- Coordinate patient care across interdisciplinary teams to ensure safe, timely discharges
- Conduct admission reviews and utilization reviews using InterQual and Medicare guidelines to determine medical necessity
- Develop and implement individualized discharge plans, arranging DME, SNF placements and ambulance transport as needed
- Document care plans and reviews accurately in Epic and other computerized charting systems
- Facilitate family meetings and participate in the development of treatment plans
- Manage appeals, denials, and benefits eligibility tasks to support patient coverage needs
- Perform care coordination for diverse patient populations including neonates through geriatrics
- Maintain regulatory compliance with CMS, DRG processes and HIPAA guidelines
Qualifications
- Active California state license required (pending license accepted but must be licensed by start date)
- Minimum 2 years of acute care case management or discharge planning experience
- Experience with travel/contract assignments required (first-time travelers can be screened)
- Proven competence with Epic or similar charting systems (computerized documentation required)
- Strong knowledge of InterQual criteria, Medicare guidelines, and admission criteria
- Ability to independently arrange DME, SNF and ambulance transport and handle complex care transitions
- Availability for every-other-weekend rotation and holiday coverage
Knowledge & Skills
- InterQual criteria and admission/review processes
- Utilization review and determination of medical necessity
- Care coordination and discharge planning best practices
- Familiarity with HMOs, IPAs, Medicare and Medi-Cal processes
- Regulatory knowledge: CMS, DRG and HIPAA
- Experience arranging DME, SNF placement and transportation logistics
- Strong documentation skills in Epic and computerized charting systems
- Ability to work with diverse patient populations and complex social determinants (homelessness, substance use, foster/adoption situations)
