Ratio: 1:25. Job involves coordinating care for patients and managing their discharge plans. Candidates will participate in the development of treatment plans. Experience with arranging durable medical equipment, skilled nursing facilities, and ambulance transport is essential. Candidates must be competent to perform these tasks independently.
