License: Valid nursing license required Certifications: BLS, Case Management Certification Must-Have: - Acute Hospital experience - Knowledge of Admission Criteria - Experience with Appeals and Denials - Care coordination skills - Familiarity with the CMS: Centers for Medicare and Medicaid Services - Ability to determine medical necessity per evidence-based guidelines - Discharge Planning experience - Understanding of DRG (Diagnosis Related Groups) - Knowledge of HEDIS (Healthcare Effectiveness Data and Information Set) measures - Compliance with HIPAA guidelines - Experience in Home Health and Hospice - Familiarity with Insurance Company practices - Knowledge related to Long-Term Acute Care/Rehab/Skilled Nursing - Understanding of NCQA (National Committee for Quality Assurance) standards - Experience with Needs Assessment/Order DME - OSHA compliance familiarity - Ability to create a Plan of Care - Knowledge of Prior Authorizations - Understanding of The Joint Commission/Core Measures/National Safety Goals Description: This position requires the ability to float between units based on patient care needs. Weekend shifts are required every other weekend. The expected patient ratio is 20 patients per case manager. Recent acute care experience is a must, particularly in the realms of hospital care management and discharge planning. Behavioral health experience does not count toward the required experience. Must possess a vehicle for this assignment. Travelers may float to various units and potentially other facilities based on skill sets. It is imperative to maintain flexibility in schedule to accommodate summer vacations and PTO utilization. Candidates should be prepared to work three out of five specified holidays each year. This role emphasizes the importance of teamwork and support from interdisciplinary teams, including social services. The daily census is approximately 320 patients. All staff members are required to adhere to the organization’s standards and protocols.
