Job Title: Care Manager, Registered Nurse Profession: RN Specialty: Care Manager, Registered Nurse Duration: 13 weeks Shift: Monday to Friday Hours per Shift: 8 am - 5 pm & Callback on weekends/holidays Experience: Minimum of 3 years clinical nursing experience License: Valid, current, full and unrestricted Professional Nursing License to practice nursing in any state of the United States Certifications: Basic Life Support (BLS) certification by the American Heart Association Must-Have: - Associate's or Bachelor's Degree in Nursing - Demonstrated knowledge of case management, contract health, and utilization review processes Description: The incumbent serves as an Outpatient/Inpatient Nurse Case Manager and acts as the supportive contact for identified high-risk patients. The focus will be on specialty clinics that require specific case management attention. The case manager meets essential health and social needs for patients while ensuring quality and cost-effective healthcare. The case manager facilitates care transitions from admission through discharge, ensuring continuity of care and alignment of services. Post-hospital referrals to continuing care agencies are also part of the role. This position serves as a resource for nursing, physicians, and ancillary departments on transitional and regulatory issues. The case manager will work with culturally diverse patient populations facing multiple barriers to self-sufficiency. These barriers may include illiteracy, welfare dependency, domestic violence, substance abuse, and mental health issues. Responsibilities include assessment, service planning, resource acquisition, monitoring progress, and responding to emerging client needs. Collaboration with an interdisciplinary team is essential to assess care plans, monitor progress, and make recommendations for safe and efficient hospitalizations. Evaluating patients for medical necessity and care coordination needs in the emergency department is required. The incumbent will assess potential admissions and work with clinicians and physicians to determine appropriate admission status. Proposing and implementing outpatient plans for safe discharge from the emergency department with physician approval is part of the role. Coordination of care for patients with abuse, chronic pain, and frequent utilization issues is expected. Interacting with patients or customers of all ages, from newborns to geriatrics, is a part of this position. Additionally, the case manager will assist individuals with mental health issues in accessing and navigating the resources necessary for improving their well-being.
