This Registered Nurse – Case Manager position is a 13-week travel assignment at Providence Health and Services in Mission Hills, CA. The RN will work day shifts (5x8 hours, 8:00 AM – 4:30 PM) and is responsible for care coordination, discharge planning, and case management in an acute care hospital setting.
Experience & Skills:
Minimum 2 years acute care hospital Case Manager experience performing care coordination and discharge planning.
Experience with InterQual criteria, DRG, and CMS regulations.
Must be able to organize and participate in family meetings, develop treatment plans, and arrange services like DME, SNF, and ambulance transport.
Must float between facilities within the state as required.
Familiarity with Epic charting system (required).
Travel experience required; first-time travelers may be screened.
