Travel Nurse RN - Case Manager - $2,710 per week in Apple Valley, CA

Job Title: Case Manager Registered Nurse Profession: Registered Nurse Specialty: Case Manager Duration: 26 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum of 2 years in acute care case management/discharge planning required. License: Current state license required (pending license accepted but must be licensed upon start date). Certifications: Valid state compact license must be in hand at submission. Must-Have: - Client experience is mandatory. - Behavioral health experience does not apply. - Must have experience with assessing patients, care coordination, discharge planning, and utilization review. - Ability to organize and participate in family meetings. - Competence in arranging DME, SNF, and ambulance transport independently. - Knowledge of HMOs, IPAs, drug/alcohol abuse treatment options, patients experiencing homelessness, and mental health issues. Description: This position requires a registered nurse specializing in case management. The role involves actively participating in discharge planning and care coordination. You will organize and conduct family meetings to discuss treatment plans. Experience with various patient demographics including neonates, adolescents, adults, and geriatrics is necessary. The daily patient census is approximately 25. You will work closely with interdisciplinary support teams including physical therapy and social services. This role demands seasoned travel experience, without floating expectations. Weekend shifts are required to meet departmental needs.
