Key Responsibilities of the RN - Case Management - Travel- Perform comprehensive assessment of high-risk members.
- Collaborate with primary care providers to ensure individualized, comprehensive care plans.
- Identify and reassess cost-efficient, appropriate levels of care.
- Facilitate non-medical support to ensure compliance with treatment plans.
- Coordinate care transitions as needed.
- Engage specialty resources for optimal resolution of cases.
- Maintain detailed records of clinical, functional, and fiscal outcomes.
- Identify opportunities for health promotion and illness prevention.
- Act as a patient advocate by protecting privacy and confidentiality.
Requirements of the RN - Case Management - Travel- Associates or Bachelor's Degree in Nursing or related field.
- Current, unrestricted state Registered Nurse license.
- Case Management certification.
- Ability to analyze complex medical information and make rational decisions.
