Responsibilities:- Coordinate and oversee patient care plans to ensure optimal health outcomes and efficient use of healthcare resources.
- Assess patients’ medical conditions, social determinants of health, and care needs to develop individualized care plans.
- Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure continuity of care.
- Coordinate insurance authorizations, appeals, and approvals for necessary medical services and treatments.
Qualifications:- CA Registered Nurse license
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)
- Basic Life Support (BLS) certification
- Case Management Certification (CCM) or Accredited Case Manager (ACM) certification preferred
Job details:- Pay: $2,780.96 - $2,866.97 / week
