Key Responsibilities of the RN - Case Management- Perform comprehensive assessment of high-risk members
- Collaborate with primary care providers for individualized care plans
- Identify cost-efficient, appropriate care levels
- Arrange non-medical support for treatment plan compliance
- Coordinate care transitions and work with a multidisciplinary team
- Engage specialty resources and maintain detailed records
- Identify health promotion and illness prevention opportunities
- Act as patient advocate, ensuring privacy and confidentiality
Requirements of the RN - Case Management- Current KY RN License or Compact RN License
- HCHB experience
- 1 year home health experience
- Associates or Bachelor's Degree in Nursing or related field
- Case Management certification
- Ability to analyze complex medical information
