Job Title: RN Case Manager – Home Health | Travel Assignment

Assignment Details:
Schedule: Monday – Friday, 8:00 AM – 4:30 PM
8-hour day shifts
Weekend coverage as needed
Position Overview:
We are seeking an experienced Registered Nurse (RN) Case Manager for a Home Health (CHHA) travel assignment with URMHC. This role focuses on delivering high-quality, patient-centered care in the home setting while coordinating treatment plans and ensuring compliance with regulatory standards.
Requirements:
Active RN License (NY preferred/required)
Certified Home Health Care experience (required)
Experience with OASIS documentation (required)
Experience with EPIC or Netsmart (required)
BLS certification (required)
Responsibilities:
Direct Patient Care:
Develop and manage individualized care plans in collaboration with patients, families, physicians, and care teams
Complete OASIS assessments, reassessments, transfers, resumptions of care, and discharges within required timeframes
Process physician orders and communicate patient status changes
Provide skilled nursing care, education, and preventive strategies
Supervise LPNs and coordinate patient care services
Maintain accurate and compliant clinical documentation
Care Coordination & Case Management:
Collaborate with multidisciplinary teams and third-party payors
Ensure continuity of care and appropriate resource utilization
Participate in patient care conferences and ongoing case reviews
Support patient-centered care management and positive outcomes
Quality & Compliance:
Ensure compliance with state and federal regulations
Participate in quality improvement initiatives and audits
Maintain productivity standards and documentation accuracy
Promote patient satisfaction and high-quality care delivery
