Registered Nurse - Case Management
Location: Carmichael, CA
Start Date: 04-28-2026
End Date: 07-28-2026
Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 4:00 PM (Day), every other weekend (at least one full weekend per month), EOW rotation; 8-hour days
Hours per Week: 40
Roles & Responsibilities
- Conduct thorough initial assessments and manage a caseload averaging 15–20 consults per day
- Coordinate patient care across the hospital, ensuring smooth transitions and timely discharge planning
- Perform utilization management and utilization review using InterQual criteria
- Document all patient interactions and care plans accurately in Cerner
- Collaborate with multidisciplinary teams (hospitalists, RT, pharmacy, social work) to advocate for patient needs
- Work effectively with all payers and support uninsured and homeless populations with tailored resources
- Participate in orientation, training, and required weekend rotations as scheduled
- Prioritize competing tasks and reprioritize throughout the day to meet patient and team needs
Knowledge & Skills
- Case management and care coordination best practices
- Utilization Management (UM) and utilization review processes
- Discharge planning workflows and community resource knowledge
- EMR proficiency (Cerner) and accurate clinical documentation
- InterQual criteria and initial assessment completion
- Strong collaboration, teamwork, and communication skills
- Ability to reprioritize quickly in a fast-paced acute care environment
- Experience working with uninsured and homeless patient populations
