Registered Nurse - Case Management

Location: Carmichael, CA

Start Date: 04-28-2026

End Date: 07-28-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 4:00 PM (Day), every other weekend (at least one full weekend per month), EOW rotation; 8-hour days

Hours per Week: 40

Roles & Responsibilities
  • Conduct thorough initial assessments and manage a caseload averaging 15–20 consults per day
  • Coordinate patient care across the hospital, ensuring smooth transitions and timely discharge planning
  • Perform utilization management and utilization review using InterQual criteria
  • Document all patient interactions and care plans accurately in Cerner
  • Collaborate with multidisciplinary teams (hospitalists, RT, pharmacy, social work) to advocate for patient needs
  • Work effectively with all payers and support uninsured and homeless populations with tailored resources
  • Participate in orientation, training, and required weekend rotations as scheduled
  • Prioritize competing tasks and reprioritize throughout the day to meet patient and team needs
Knowledge & Skills
  • Case management and care coordination best practices
  • Utilization Management (UM) and utilization review processes
  • Discharge planning workflows and community resource knowledge
  • EMR proficiency (Cerner) and accurate clinical documentation
  • InterQual criteria and initial assessment completion
  • Strong collaboration, teamwork, and communication skills
  • Ability to reprioritize quickly in a fast-paced acute care environment
  • Experience working with uninsured and homeless patient populations
