This role is responsible for care coordination and care transitions planning throughout the acute care patient experience. The Case Manager works in collaboration with physicians, utilization managers, medical social workers, and bedside RNs. The goal is to assure timely progression and transition of patients to the appropriate level of care. This aims to prevent unnecessary admissions or readmissions. The care management process encompasses communication and facilitates care across the continuum through effective resource coordination. The main objectives include achieving optimal health, access to care, and appropriate resource utilization. This must be balanced with the patients' self-determination. The Case Manager collaborates with patients, families, physicians, and the interdisciplinary team. The role also includes coordination with nursing management, quality assurance, ancillary services, and third-party payers. In the emergency department, the care management process focuses on addressing complex clinical and social situations efficiently. Job Responsibilities: Conduct patient assessments and continued evaluations. Review initial physician admission care plans. Gather additional medical, psychosocial, and financial information through interviews and medical records. Determine the risk level for readmission. Conduct screenings for ancillary supportive services, including palliative care needs. Supervise and lead the healthcare team in developing cost-effective care coordination plans. Identify and refer quality and risk management concerns. Direct and oversee case management assistants in determining preferences for post-acute care services. Review medical records to ensure that patients continue to meet level of care requirements. Work with attending physicians to confirm necessary documentation. Facilitate transition planning for patients no longer requiring acute care. Monitor length of stay and address outliers requiring additional resources or focus. Collaborate with financial counselors regarding inpatient stay denials. Deliver necessary information in compliance with regulations. Participate actively in patient rounds and collaborate with interdisciplinary teams. Document consistently in the electronic health record and maintain knowledge of regulatory requirements. Formulate transition plans while collaborating with patients, families, and the healthcare team. Proactively identify barriers to care progression and work with multidisciplinary teams to resolve them. Review and modify the plan of care as necessary. Communicate follow-up appointment needs to patients and families prior to transition. Ensure necessary paperwork for post-acute transfers complies with regulations. Identify high utilizers and make appropriate care plans and referrals to community resources. Develop and maintain positive relationships with the healthcare team and community agency representatives. Contribute actively to department operations and process improvements. Engage positively and constructively with colleagues and the leadership team. Utilize effective communication skills to promote customer service. Relate with respect and consideration to all customers. Maintain an engaged and constructive approach toward departmental operations. Provide and accept constructive feedback among colleagues. Use effective communication skills to resolve issues promptly.
