Travel Nurse RN - Case Manager - $2,832 per week in Eureka, CA

Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 26 weeks Shift: Day 5x8-Hour Hours per Shift: 8:00 AM - 4:30 PM Experience: Minimum of 2 years required License: Valid state compact license required Certifications: Current relevant certifications required Must-Have: - Acute care experience - Experience with health maintenance organizations and independent practice associations - Experience with advance directives, POLST, and protective services process Description: This position requires the candidate to work every other weekend. Candidates with acute care experience are highly preferred. First-time travelers will be accepted with clinical approval after screening. Call is not required for this position. Candidates may need to float between like units based on departmental needs. Local candidates will be accepted as well. All requested time off (RTO) must be included at the time of submission. The workload can include a caseload of up to 25 patients. It is important for candidates to have experience in case management, with skills in care coordination, discharge planning, and appeals and denials management. Knowledge of health insurance and eligibility requirements is crucial. The role may involve collaboration with interdisciplinary support including social services. The candidate should have experience with CMS regulations and national safety goals. References are required to verify prior experience. All candidates should prepare to work three of the five major holidays during the duration of their contract. Additional training will be provided to help candidates familiarize themselves with necessary documentation and charting processes. Travelers may also be expected to assist patients in diverse settings, including pediatrics and geriatrics.
