Job Title: Registered Nurse Non-Certified Case Manager Profession: RN Specialty: Care Manager, Med-surg Adult Duration: 13 Weeks Shift: Day Shift Hours per Shift: 8 (7:30 AM - 4:30 PM) Experience: Minimum of 2 years of nursing experience License: Required Montana Licensure (Registered Nurse) Certifications: American Heart Association BLS Must-Have: Utilization review experience Description: The Registered Nurse Non-Certified Case Manager works with physicians and multidisciplinary team members. Develops a plan of care for assigned patients. Ensures the patient is progressing towards desired outcomes by monitoring care through assessments and/or patient records. Identifies and resolves barriers that hinder effective patient care. Actively involved in the discharge planning process. Completes comprehensive assessments of members per regulated timelines. Determines who may qualify for case management based on clinical judgment and changes in the member's health or psychosocial wellness. Develops and implements a case management plan in collaboration with the member, caregiver, physician, other healthcare professionals, and the member's support network. Performs ongoing monitoring of the care plan to evaluate effectiveness. Documents interventions and goal achievement, and suggests changes accordingly. Maintains an ongoing member caseload for regular outreach and management. Promotes integration of services for members to enhance continuity of care. Demonstrates sound judgement, patience, and maintains a professional demeanor at all times. Ability to work in a busy and stressful environment. Strong interpersonal, verbal and written communication skills are essential. Exercises tact, discretion, sensitivity, and maintains confidentiality. Proficiency in computer applications, including MS Office and EMR. Ability to analyze, organize, and prioritize work while meeting multiple deadlines. Strong emotional intelligence and teamwork skills are necessary. Regular and sustained attendance is required for this role. May necessitate working beyond a standard workweek, including weekends and after-hours shifts. On-call work may be required to respond promptly to organizational needs. Physical requirements include lifting up to 50 pounds, maintaining sitting and standing postures for extended periods, and good balance and coordination while walking. Cognitive skills include learning new tasks, remembering processes, maintaining focus, and making timely decisions. Exposures include bloodborne pathogens, various chemicals, and emotionally challenging situations.
