The Case Manager RN will be responsible for managing patient care through effective care coordination and discharge planning in an acute hospital setting. The role involves collaborating with the interdisciplinary team, assessing patient needs, facilitating safe transitions of care, and ensuring compliance with hospital and regulatory standards.
