We are seeking a dedicated and experienced Registered Nurse (RN) – Case Manager to provide comprehensive care coordination, particularly for patients with neuro-oncology needs. The RN Case Manager will play a key role in developing and managing individualized care plans, ensuring seamless coordination across services and providers.
Key Responsibilities- Develop and manage customized patient care roadmaps
- Coordinate follow-up appointments and ongoing care services
- Order and track labs, imaging, and diagnostic tests
- Facilitate referrals and obtain necessary authorizations
- Coordinate Durable Medical Equipment (DME) orders and delivery
- Provide end-to-end care coordination across multidisciplinary teams
- Lead transition planning (e.g., hospital to home or other care settings)
- Conduct patient needs assessments and adjust care plans accordingly
- Work closely with neuro-oncology patients and care teams
