Description
Duration: 13 Weeks
Shift: Days | Monday–Friday | 8:00 AM – 4:30 PM
- Inpatient discharge planning
- Coordinate patient discharges to home, SNF, community, or other facilities
- Work alongside Social Workers and physicians
- Manage approximately 17–25 patients
- Attend interdisciplinary rounds
- Make referrals for services such as dialysis, wound vacs, housing, shelters, and more
