• Lead the way in coordinating care services for assigned patients throughout their episode of illness or treatment.
• Utilize your advanced clinical skills to assess, plan, implement, and evaluate care processes.
• Collaborate closely with patients, families, physicians, and multidisciplinary teams to ensure the best outcomes.
• Promote efficient use of resources and community services while achieving optimal clinical results.
• Engage in quality and utilization analysis, focusing on readmission criteria and comprehensive patient care.
• Be a champion for patient advocacy, maintaining open lines of communication with all parties involved.
• Possibility of assignments in the Emergency Department to streamline care during critical moments.
• Remain actively involved from admission until discharge or appropriate transfer to a different level of care.

• Minimum of one year of recent Case Management experience in an acute care setting.
• Current California RN Licensure.
• Current BLS/BCLS certification.

8-hour shifts, 8AM - 4:30PM, including weekends and weekdays.
