Oversees the management of acute patient populations across the care continuum. Focuses on providing coordinated and integrated care to prevent unnecessary admissions or readmissions. Provides discharge planning coordination and intervention for high-risk patients presenting to the emergency department. Strives to promote patient wellness and improved care outcomes. Ensures efficient utilization of health services and minimized denials of payment for patients with complex health needs. Utilizes InterQual criteria and Milliman guidelines in care coordination. Conducts concurrent review, continued stay reviews, and retrospective reviews for medical necessity. Knowledge of CPT coding and billing, DRG, HEDIS Measures, and compliance with regulatory standards is required. Maintains adherence to HIPAA guidelines and ensures patient confidentiality. Collaborates with interdisciplinary teams to support patient care. Daily census of 100-120 patients, managing multiple visits throughout the day. Rotational weekend coverage required. Ability to float between inpatient and emergency department coverage as necessary. Complies with all work history verification requirements. Demonstrates proficiency in specific charting systems, preferably Epic. Possesses a strong understanding of patient admission criteria and appeals/denials processes.
