Description:
This position will support complex cases within the healthcare system and patients in the community virtually.
The role may require floating between different offices but will primarily be performed virtually.
Candidates must have a minimum of two years of experience in case management.
Experience in inpatient settings is necessary.
The ideal candidate will have a solid understanding of care coordination, discharge planning, and disease management.
Familiarity with CMS guidelines and utilization review processes is essential.
Strong communication skills are a must, as the role involves coordinating with multiple stakeholders.
BLS certification and a valid California RN license are required for consideration.
