Our team focuses on skilled nursing and therapy services in the home setting. We provide care for a variety of patient types and common diagnoses including CHF, COPD, diabetes, post-surgical care, wound care, medication management, and fall risk/deconditioning. The typical productivity model involves 5-6 visits per day, depending on patient acuity and geography. Candidates should possess strong clinical assessment skills, wound care experience, medication management experience, and OASIS documentation experience is preferred. Support is available within the department including CNAs assigned to field RNs, scheduling and coordination team support, and clinical leadership support. Physicians and hospitalists are accessible via standard communication pathways. The EMR used is Epic, and standard home health equipment is provided. Travel may be required within a designated coverage area and may include supporting neighboring teams if needed. Scheduling involves generally daytime hours with flexible start and end times based on patient visits. There are no weekend rotations, on-call requirements, or holiday expectations. Professional attire is expected in this field-based role.
