Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum 2 years of experience required License: Current CA License required. Certifications: Relevant certifications required. Must-Have: Inpatient Case Management experience required. Preferred Charge Experience. Experience with Epic charting system required. Community hospital experience required. Travel experience required. Description: This position will support complex cases across the system and patients in the community virtually. The role may require floating between offices but will primarily be remote. Candidates should have experience in care coordination and discharge planning. Knowledge of CMS guidelines and HIPAA regulations is essential. Candidates will be expected to demonstrate skills in acute hospital care and disease management. Experience with medical necessity and prior authorizations is necessary. The ability to utilize InterQual criteria and engage in concurrent reviews is important. Interdisciplinary support, including social services and interpretation services, will be available. This position is focused on adult and geriatric patient populations. Candidates should provide references obtained within the last year. All compliance documents must be submitted before the start date. Work history verification for the last 7 years is required. Candidates should be able to manage patient ratios effectively. This role will involve telephonic communication as part of care coordination.
