Travel Nurse RN - Case Manager - $3,192 per week in Carmichael, CA

Job Title: Case Manager Profession: Registered Nurse (RN) Specialty: Care Coordination/Case Management Duration: 17 weeks Shift: 8H Days Hours per Shift: 8 hours Experience: Minimum of 2 years License: Required Certifications: BLS Certification Must-Have: - Recent experience in Care Coordination or Case Management - Familiarity with Interqual criteria - Strong collaboration and teamwork skills - Effective communication abilities - Capacity to reprioritize tasks throughout the day - Experience dealing with all payers, uninsured, and homeless populations Description: This position involves case management and care coordination responsibilities. Case managers will complete initial assessments and discharge planning. They will work with a caseload of 15-20 patients daily, performing consults as required. Utilization review is a necessary component of this role. Candidates will be trained in Interqual criteria and processes. The position may require working every other weekend, with at least one full weekend per month. Holidays are not mandatory, but willingness to work during holidays is valued. Collaboration with a multidisciplinary team is essential for success in this role. Candidates will encounter a variety of patients, requiring adaptability and effective communication skills. Experience with Cerner or similar charting systems is preferred.
