Certifications: - Associate Degree in nursing required. Bachelors of Science in Nursing (BSN) preferred.
- Current California Registered Nurse license required.
- Current BLS/Healthcare Provider status as per American Heart Association standards required.
- BLS - AHA
- California Registered Nurse License
Mandatory Experience: - Transitional Care Coordinator
- 3 years’ nursing experience required.
- Ability to demonstrate a working knowledge of community resources, post-acute care coordination, and case management principles required.
- Bilingual in Spanish preferred.
- Case Management experience preferred.
- Broad general knowledge of nursing and possess the ability to effectively navigate and utilize a computerized medical record system.
- Epic experience needed ( recent)
- Case Management experience desired
- Shift: Monday – Friday-13 weeks 7:30a – 4p
