Registered Nurse - Case Management
Location: Everett, WA
Contract Type: Travel Contract
Shift Type: 4x8 Day schedule, 7:30 AM - 4:00 PM, 4 shifts per week, Day shift, every other weekend required
Hours per Week: 40
Candidate Pay Rate: $64 per hour

Roles & Responsibilities
- Coordinate discharge planning and care coordination to ensure smooth patient transitions
- Perform needs assessments and order DME when appropriate to support safe discharge
- Collaborate with interdisciplinary teams, including social services, to develop plans of care
- Utilize EPIC for accurate charting and documentation of case management activities
- Support utilization review and CMS-related regulatory activities as needed
- Float across hospital areas including Colby and Pacific campuses to meet patient needs
Qualifications
- Active RN license required for nursing applicants; MSW accepted for social worker applicants
- BLS certification required
- Minimum 1 year of acute care experience, with discharge planning/care coordination experience
- Experience in case management or utilization review is required
- Comfortable working across Med Surg, ED, ICU settings is strongly preferred
- Pending license accepted; please include all RTO restrictions at time of submittal
Education
- Registered Nurse (RN) credential for nurses or Master of Social Work (MSW) for social worker applicants
- BLS certification
Knowledge & Skills
- Discharge planning and care coordination expertise
- Acute care and case management experience
- Proficient with EPIC charting system
- Familiarity with CMS regulations and utilization review processes
- Skilled in needs assessment, plan of care development, and ordering DME
- Ability to manage high patient ratios and prioritize effectively
