Location: Anchorage, AK

Start Date: 05-11-2026

End Date: 08-10-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, Day shift. Start times vary; one of: 7:00 AM - 3:30 PM, 7:30 AM - 4:00 PM, 8:00 AM - 4:30 PM, or 8:30 AM - 5:00 PM. Every other weekend required (or 4 weekend days in a 4-week schedule).

Hours per Week: 40

Candidate Pay Rate: $74 per hour

Roles & Responsibilities
  • Coordinate comprehensive case management and discharge planning to ensure safe, timely transitions from the hospital
  • Perform utilization review using Milliman Guidelines to support appropriate care decisions
  • Collaborate with multidisciplinary teams to create and manage individualized plans of care
  • Conduct needs assessments, arrange DME orders, and connect patients with community resources
  • Document thoroughly and accurately in Epic to maintain continuity of care and regulatory compliance
  • Support care coordination across services including PT, RT, pharmacy, social work, and interpreters
  • Float within case management services as needed to cover gaps and support the team
Qualifications
  • AK RN license in hand required for consideration
  • Minimum 1 year of acute care experience
  • Epic experience required (hardstop may apply)
  • Experience in case management or utilization review preferred
  • ACM-RN certification preferred
  • BLS certification preferred
  • Ability to work every other weekend and float within the scope of case management services
  • Comfort working in a Magnet, Level II Trauma acute hospital environment
Knowledge & Skills
  • Strong knowledge of case management and utilization review processes
  • Familiarity with Milliman Guidelines and CMS/HIPAA regulatory expectations
  • Proficient charting and documentation in Epic
  • Care coordination and discharge planning expertise
  • Ability to perform needs assessment and order DME
  • Good communication and interdisciplinary collaboration skills
  • Basic computer skills including Microsoft Office and Teams
