Shift Type: Monday-Friday, Day shift. Start times vary; one of: 7:00 AM - 3:30 PM, 7:30 AM - 4:00 PM, 8:00 AM - 4:30 PM, or 8:30 AM - 5:00 PM. Every other weekend required (or 4 weekend days in a 4-week schedule).

Roles & Responsibilities
- Coordinate comprehensive case management and discharge planning to ensure safe, timely transitions from the hospital
- Perform utilization review using Milliman Guidelines to support appropriate care decisions
- Collaborate with multidisciplinary teams to create and manage individualized plans of care
- Conduct needs assessments, arrange DME orders, and connect patients with community resources
- Document thoroughly and accurately in Epic to maintain continuity of care and regulatory compliance
- Support care coordination across services including PT, RT, pharmacy, social work, and interpreters
- Float within case management services as needed to cover gaps and support the team
Qualifications
- AK RN license in hand required for consideration
- Minimum 1 year of acute care experience
- Epic experience required (hardstop may apply)
- Experience in case management or utilization review preferred
- ACM-RN certification preferred
- BLS certification preferred
- Ability to work every other weekend and float within the scope of case management services
- Comfort working in a Magnet, Level II Trauma acute hospital environment
Knowledge & Skills
- Strong knowledge of case management and utilization review processes
- Familiarity with Milliman Guidelines and CMS/HIPAA regulatory expectations
- Proficient charting and documentation in Epic
- Care coordination and discharge planning expertise
- Ability to perform needs assessment and order DME
- Good communication and interdisciplinary collaboration skills
- Basic computer skills including Microsoft Office and Teams
