Job Title: Nurse Educator, Care Manager Profession: Nursing Specialty: Nurse Educator, Care Manager Duration: Full Time Shift: Days Hours per Shift: M-F 8-5 Experience: 1 year License: Registered Nurse License - State of Alaska Certifications: Certified Diabetes Educator - National Certification Board for Diabetes Educators, Clinical Competency Assessment Must-Have: - Valid, current RN license in the state of Alaska - Certification as a Diabetic Educator or eligibility to apply for certification within 2 years - Clinical Competency required within 3 months of hire and every three years Description: Responsible for maintaining a formal comprehensive diabetes education program encompassing medical and clinical providers, physical therapists, mental health practitioners, and health educators. Monitors equipment that provides eligible patients with the tools and support to self-manage their diabetes, thus reducing chronic care costs. Provides education and case management to individuals with diabetes, prediabetes, and gestational diabetes. Receives and relays messages to personnel and uses proper telephone etiquette. Experience required includes general nursing or dietetic experience, preferably with one year in diabetic care and management. Knowledge of diabetes case management, practices, and procedures is essential to assess patient care. Familiarity with health promotion and patient education activities is necessary. Appreciation of cultural differences and their effect on health care delivery is important. Skills required include performing a variety of responsible diabetic care procedures, employing tact and sensitivity in stressful situations, and effective oral and written communication. Ability to work independently in completing tasks and lead group programs is essential. Engaging patients in self-management activities and providing positive feedback is expected. Must prioritize tasks and use good judgment while coordinating various issues, often with interruptions. Physical demands include regularly talking or hearing, frequently standing, walking, and sitting, using hands to handle or feel, and occasionally climbing or balancing, stooping, kneeling, crouching, or crawling. The ability to lift and/or move 50 lbs is required.
