Requirements:
- Active Registered Nurse (RN) license
- Diabetes education or case management experience preferred
- Certified Diabetes Care and Education Specialist (CDCES) preferred or willingness to obtain
- BLS certification required
- Strong patient education and care coordination skills
- Excellent communication and organizational abilities
Responsibilities:
- Provide individualized diabetes education and self-management training
- Assess patients’ clinical needs and develop care plans
- Monitor blood glucose trends and treatment adherence
- Coordinate care with providers and interdisciplinary teams
- Educate patients on medications, insulin administration, nutrition, and lifestyle changes
- Document patient progress and outcomes accurately
- Support population health and chronic disease management initiatives
