RN Care Coordinator

POSITION SUMMARY RN Case Manager supporting colorectal surgery patients through pre-op and post-op care coordination.
POSITION DUTIES
- Coordinate pre-op and post-op patient care
- Schedule surgeries, imaging, and follow-up appointments
- Collaborate with providers and care teams
- Document care in Epic
MINIMUM REQUIRED QUALIFICATIONS - Active RN license (MN required)
- 5+ years of RN experience
- Previous RN CM Experience
- Epic experience required
PREFERRED QUALIFICATIONS - Surgical experience preferred
- Med-Surg background preferred
- Colorectal surgery experience a plus
LENGTH OF ASSIGNMENT 12–20 Weeks
SHIFT / HOURS PER WEEK Monday–Friday, Onsite
40 Hours per Week
