POSITION SUMMARY – Case Management Assistant

POSITION DUTIES – Supports the care transition and discharge planning process by coordinating referrals and placements with post-acute providers, obtaining insurance information and authorizations, preparing discharge documentation and patient transfer packets, arranging transportation, conducting bed searches, communicating placement updates and barriers to the care team, assisting with Medicare notices and appeals, scheduling post-discharge appointments, maintaining community resources, managing communications, and ensuring compliance with organizational policies, payer requirements, and regulatory standards while performing other assigned duties to support efficient, high-quality patient care.

MINIMUM REQUIRED QUALIFICATIONS –

• High school diploma, GED or higher level degree
• Epic knowledge
• Knowledge of payer types including Medicare, SR HMO, Medi-Cal and Private Payers
• Ability to use Microsoft Office Suite (Word, Excel, Outlook, PowerPoint) and other relevant software applications

PREFERRED QUALIFICATIONS –

• Associate's degree
• Medical Assistant, Certified Nursing Assistant, Medical Terminology or other health related certification

LENGTH OF ASSIGNMENT – 4 weeks with possibility of extension

SHIFT / HOURS PER WEEK – M-F, 8a-4:30p

SYSTEMS – Epic

START DATE – ASAP
