Description

American Traveler is hiring an RN Case Manager for an inpatient Care Management role requiring RN CM certification and prior utilization review experience.

Details

• RevCycle - Care Management unit within an acute care hospital setting
• Day shift, 8:00 AM-4:30 PM, Sunday through Thursday, 40 hours per week
• No on-call requirements
• Manages a heavy caseload including complex patients, frequent ED utilizers, chronic pain, substance abuse, and homeless patient populations
• Coordinates continuum of care from pre-admission through post-discharge

Requirements

• Active CA RN license required
• BLS certification from an approved vendor required
• RN Case Management certification required
• Inpatient case management experience required, preferably with strike force experience for KP or HCA CA facilities
• BSN preferred
• Case management experience preferred

Additional Information

• Leads coordination of patient care across disciplines, monitoring appropriateness and timeliness of care
• Collaborates with physicians on resource utilization, treatment plans, length of stay, and discharge planning
• Coordinates transfers to tertiary centers including transfer of patient information for continuity of care
• Collects and manages data required for regulatory and accreditation compliance
• Candidates with more than 5 days of requested time off will be automatically declined
• Candidates with any prior adverse action on their license will not be considered
• Accepts local candidates at the same rate as travel candidates with no radius restriction
