This is a Nurse Educator role focused on coordinating a Diabetes Education Program within an outpatient clinic setting.
You will lead patient education initiatives while overseeing clinical staff and ensuring high-quality diabetes care and program delivery.
Coordinate and manage the Diabetes Education Program in an outpatient setting.
Provide education to patients on diabetes management, treatment plans, and lifestyle modifications.
Supervise a team including 2 RNs and 1 Dietician.
Monitor patient progress and ensure effective care outcomes.
Perform administrative duties including documentation, reporting, and program oversight.
Collaborate with providers and interdisciplinary teams to optimize patient care.
