Job Summary: The Care Manager - ED is responsible for providing care coordination, utilization review, and discharge planning. This role requires a strong clinical background and the ability to work collaboratively with healthcare teams to ensure optimal patient outcomes.
Required Qualifications:
- Five (5) years minimum of clinical experience as a Registered Nurse (RN).
- Experience with care coordination, utilization review, and discharge planning.
- Computer literacy.
- Associates Degree (ASN, ADN) from an accredited nursing program.
- RN license as a Registered Nurse in the State of New York.
- PRI and Screen certification within three months of employment.
- Primary source verification of license and registration of Registered Professional Nurse from NYS Department of Education (NYSED).
- BLS certification.
Preferred Qualifications:
- Leadership experience.
- Bachelor's or Master's degree in Nursing.
- Case Management Certification.
