Weekend Field Registered Nurse (RN) - PACE Program
Job Summary
We are seeking a dedicated and compassionate Weekend Field Registered Nurse (RN) to join our Program of All-Inclusive Care for the Elderly (PACE) team in Battle Creek, Michigan. This vital role focuses on providing comprehensive, participant-centered nursing care to frail, elderly, and long-term care individuals within their homes and coordinating with an interdisciplinary team.
The successful candidate will conduct in-person assessments to determine appropriate levels of care, deliver essential nursing services such as wound care, IV hydration, and lab draws, and respond promptly to communications from primary care providers and support staff. This position requires a commitment to quality improvement, meticulous documentation, and effective collaboration with various healthcare stakeholders to ensure holistic participant support.
Key Responsibilities
Perform in-person participant assessments to determine appropriate levels of care (emergent, urgent, non-urgent, home care status) according to the participant's plan of care.
Respond promptly to calls from Primary Care Providers (PCP), triage, and SCPP support staff, prioritizing needs as calls are received.
Conduct regular field visits to provide direct nursing care, including wound care, IV hydration, lab draws, and comprehensive nurse assessments.
Utilize full body assessment skills, critical thinking, and Electronic Medical Records (EMR) for documentation during field visits.
Provide education, instruction, and support to participants, Durable Power of Attorney (DPOA), family, friends, and/or facility staff.
Coordinate and collaborate with providers, SCPP staff, Durable Medical Equipment (DME) suppliers, Assisted Living Facilities (ALF), Skilled Nursing Facilities (SNF), paramedics, pharmacy, administration, home care coordinators, hospitals, and other partners.
Initiate emergency treatment if necessary and document new orders, including updating Pressure Ulcer Incidence/Injury (PUI) records.
Complete appropriate documentation for incidents such as falls (with/without injury), threats of suicide, medication errors, incident reports, grievances, and other quality measure policies.
Actively participate in new employee orientation, ongoing in-service programs, staff meetings, continuous quality improvement initiatives, and performance/protocol evaluations.
Adapt to changes in workload and schedules based on evolving team priorities and maintain effective communication with leadership.
Uphold a participant-centered care model, collaborating with all individuals involved in the participant’s care.
Required Qualifications
Associate's Degree in Nursing from an accredited School of Nursing or University.
Minimum of one (1) year of experience working with a frail, elderly, or long-term care population.
