Job Summary

We are seeking a dedicated and compassionate Certified Nursing Assistant (CNA) for a Skilled Nursing Facility (SNF) in Alturas, California. This role is integral to providing high-quality, individualized care to residents within a long-term care setting.

The successful candidate will work collaboratively with nursing staff, residents, families, and various facility departments. Key responsibilities include direct patient care, resident monitoring, and ensuring timely communication regarding resident conditions to optimize overall well-being and support a positive living environment.

Key Responsibilities

• Receive and provide comprehensive reports on resident conditions, ensuring clear understanding of care plans.
• Conduct routine rounds on all residents, performing initial assessments for new admissions, ill residents, or those who have experienced falls or injuries.
• Accurately document resident assessments and care activities in accordance with facility policies.
• Promptly notify nursing staff of any changes in resident conditions or immediate needs.
• Assist residents with activities of daily living and provide direct patient care, ensuring call lights are answered in a timely manner.
• Foster positive working relationships with co-workers, other departments, residents, families, and physicians.

Required Qualifications

• Active California Certified Nursing Assistant (CNA) license.
• Experience working in a Skilled Nursing Facility (SNF) or Long Term Care (LTC) environment.
