Licensed Practical Nurse (LPN), Long-Term Care
Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for a contract assignment within a Long-Term Care (LTC) facility located in Meeker, Colorado. This role is crucial for providing high-quality, direct patient care to residents requiring ongoing support and medical attention.
The successful candidate will be an integral part of our healthcare team, focusing on the well-being and daily needs of our patient population. Responsibilities will include medication administration, monitoring patient conditions, and collaborating with interdisciplinary staff to ensure comprehensive care plans are executed effectively.
Key Responsibilities
Provide direct nursing care to residents in accordance with established policies, procedures, and physician orders.
Administer medications and treatments accurately and timely.
Monitor, assess, and document changes in resident conditions, reporting significant findings to the Registered Nurse or physician.
Assist residents with activities of daily living (ADLs) as needed.
Maintain accurate and thorough electronic health records (EHR) and other required documentation.
Collaborate effectively with the healthcare team, including RNs, CNAs, and other support staff, to ensure coordinated patient care.
Educate residents and their families on health maintenance and disease prevention as appropriate.
Required Qualifications
Active, unrestricted Licensed Practical Nurse (LPN) license in the state of Colorado.
Minimum of 1 year of recent experience in a Long-Term Care (LTC) setting.
Current Basic Life Support (BLS) certification.
Preferred Qualifications
Experience with Minimum Data Set (MDS) assessments.
