**Join Our Team as an LPN in Acute Care!**
**Job Description:**
- Collaborate with an interdisciplinary team under the guidance of a professional Registered Nurse.
- Deliver skilled nursing care and administer medications to patients in need.
- Participate in the nursing process: assessment, planning, implementation, and evaluation of patient care.
- Provide direct nursing care and assistance following directives from healthcare professionals.
- Contribute valuable insights to the patient's plan of care through teamwork with patient/family and healthcare team.
- Conduct patient data collection and observations tailored to diverse patient needs.
- Document objective and subjective data accurately in compliance with hospital guidelines.
- Ensure patient care is delivered safely, knowledgeably, and consistently.
- Administer medications and treatments, using appropriate medical equipment.
- Perform phlebotomy services as required.
- Evaluate patient responses to nursing interventions and adjust care plans accordingly.
- Recognize and report significant changes in patient conditions.
- Maintain a focus on patient safety and adhere to organizational policies.
- Participate in ongoing training and development to enhance skills and knowledge.
**Qualifications:**
- Current Licensed Practical Nurse (LPN) license or applicant status in the state of employment.
- Basic Life Support (BLS) certification from the American Heart Association.
- Preferred: One year of clinical patient care experience in an acute care setting or long-term care facility.
- Preferred: EPIC Electronic Health Record (EHR) training.

- Ability to lift and carry up to 100 lbs, and to push/pull similar weights.
- Manual dexterity and a good standard of visual acuity required.
