Description:
This position delivers coordinated nursing care for a patient or an assigned group of patients according to established standards of care and the nursing process.
The role involves supervising and directing the activities of various levels of assigned nursing staff.
Coordination of care with other disciplines is essential while utilizing critical thinking, professional and supervisory discretion, and independent judgment.
The nurse collects relevant data pertinent to the patients’ health or situation.
Assessment data is analyzed to determine diagnosis and care issues.
A plan that prescribes interventions to attain outcomes is developed.
The plan is implemented, care delivery is coordinated, and strategies are employed to promote health in a safe environment.
Progress toward attaining outcomes is evaluated.
Outcomes for the patient or the patient’s situation are identified.
Collaboration with the team of patient, family, and healthcare providers is critical in providing patient care in a safe and caring environment.
This position provides learning opportunities for patients, family members, and team members.
Health information is directly provided to patients, families, and the treatment team.
Participation in discharge planning ensures continuity of care.
Appropriate delegation and coordination of duties for healthcare team members are essential.
