Registered Nurse - Community Health

Job Summary

Join a dedicated healthcare team in Rockford, Illinois, focused on empowering individuals through person-centered care. This Registered Nurse (RN) position offers a unique opportunity to provide comprehensive medical and psychiatric oversight within a community-based setting, emphasizing professional autonomy and collaborative care coordination. You will serve as a vital anchor, assessing and monitoring the health and well-being of individuals, free from the pressures of traditional hospital metrics.

This is a 13-week contract assignment, providing an immersive experience in a supportive environment where your expertise will directly impact the lives of those you serve. You will work closely with a multidisciplinary team, including community healthcare providers, nutrition services, and behavior specialists, to deliver holistic and high-quality care.

Key Responsibilities
  • Collaborate effectively with community healthcare providers, nutrition services, and behavior specialists to ensure integrated, holistic patient care.
  • Oversee and coordinate all healthcare appointments, ensuring timely scheduling, follow-ups, and comprehensive documentation.
  • Provide training, empowerment, and delegation of medication administration and other nursing tasks to the Direct Support Team, adhering strictly to state licensing regulations and organizational policies.
  • Maintain, review, and audit medical records to monitor health concerns, track patient progress, and accurately document all delegated nursing tasks.
Required Qualifications
  • Active and unencumbered Registered Nurse (RN) license.
  • Minimum of one (1) year of clinical registered professional nursing experience within the last five years.
  • Ability to perform frequent standing, walking, bending, and reaching (overhead, shoulder, and below knee level).
  • Ability to occasionally perform kneeling, climbing, stooping, crouching, and balancing.
  • Ability to occasionally lift up to 25 lbs independently, push/pull up to 40 lbs of force, and safely transfer up to 50 lbs.
Preferred Qualifications
  • Bachelor of Science in Nursing (BSN).

  • Strong patient assessment and monitoring skills for medical and psychiatric care.
  • Proficiency in care coordination and managing healthcare appointments.
  • Ability to effectively train, empower, and delegate tasks to support staff.
  • Excellent medical record management, auditing, and documentation skills.

Shift Details
  • Shift Type: AM (Day hours)
  • Shift Duration: 8 hours per day
  • Days per Week: 5 days (Monday-Friday)
  • On-Call Requirements: Every 3rd week, primarily phone-based for emergencies with rare in-person response. Supported by a robust network of supervisors, QIDPs, and an RN Supervisor.
  • Contract Length: 13 weeks
  • Start Date: 2026-07-21
Licensing Requirements
  • Active and unencumbered Registered Nurse (RN) license in the State of Illinois.
