Job Title: Care Manager Profession: Care Manager Specialty: ED Duration: 8 months Shift: 7 hours Hours per Shift: 5 days a week Experience: Minimum of five years of clinical experience as a Registered Nurse License: RN licensed as a Registered Nurse in the State of New York required Certifications: Baccalaureate degree required Must-Have: Care Coordination, Utilization review, and discharge planning experience required Description: The primary goal of the Care Manager is to optimize members’ health care and delivery of care experience. The role is expected to achieve cost savings due to improved quality of care. This is accomplished through engagement and understanding of the member’s needs, environment, providers, and support system. The Care Manager is expected to assess and evaluate the member’s needs and be a creative, efficient, and resourceful problem solver. In collaboration with the member’s care team, a plan of care with individualized goals and interventions is developed, implemented, and outcomes evaluated. The ability to proficiently read and interpret medical records, claims data, pharmacy, lab reports, and prescriptions is required. Minimum of two years of prior experience in a health care setting, Care Management, or Managed Care setting is required. Leadership experience is preferred. Computer literacy is required.
