Job Summary

We are seeking a dedicated and compassionate Registered Nurse (RN) to provide comprehensive nursing care on a Geropsychiatric Unit in Nashua, New Hampshire. This role involves working with an adult and geriatric patient population experiencing various psychiatric conditions, focusing on their mental and physical well-being within a therapeutic environment.

The successful candidate will be integral to a multidisciplinary treatment team, responsible for assessing, monitoring, and managing patient conditions, administering treatments, and facilitating therapeutic activities. This position requires a commitment to patient-centered care, meticulous documentation, and collaborative practice to ensure optimal patient outcomes and support efficient discharge planning.

Key Responsibilities

• Execute comprehensive nursing care activities for geriatric psychiatric patients.
• Assess, monitor, record, and report changes in patient conditions, including mental and physical status.
• Maintain accurate patient reports, medical histories, and all required documentation in a timely manner (e.g., admission paperwork, assessments, progress notes, interventions, evaluations, discharge paperwork).
• Administer medication and treatments as per physician orders, observing and documenting patient reactions.
• Provide assistance to patients with daily living activities and support their therapeutic engagement.
• Manage and facilitate activities within the therapeutic milieu, including leading daily groups, providing patient education, and conducting one-on-one sessions.
• Collaborate effectively with physicians, multidisciplinary team members, and external health and social agencies to develop and implement individualized treatment plans.
• Plan, implement, and coordinate nursing care activities, ensuring adherence to therapeutic standards and regulatory guidelines.
• Conduct therapies in accordance with patient diagnoses and individual needs.
• Develop and facilitate efficient discharge plans, ensuring continuity of care.
• Assure quality of care by measuring health outcomes against patient care goals and regulatory standards.

Required Qualifications

• Education: Associate's Degree in Nursing (ADN) or a diploma from a 3-year Nursing program.
