Job Summary

Join a dedicated team as a Registered Nurse (RN) Case Manager at a hospital facility in Chillicothe, Ohio. This role focuses on providing comprehensive case management services for acute medical and surgical patients, ensuring optimal patient outcomes and efficient transitions of care.

The successful candidate will be responsible for conducting thorough assessments of discharge needs, addressing social determinants of health, and coordinating effective discharge planning and referrals to ensure continuity of care. A typical daily caseload involves approximately 20 patients.

Key Responsibilities

• Conduct comprehensive assessments of patient discharge needs for acute medical and surgical populations.
• Identify and address social determinants of health impacting patient care and discharge planning.
• Develop and implement individualized discharge plans, coordinating referrals to appropriate post-acute care services and community resources.
• Collaborate effectively with interdisciplinary teams, patients, and families to facilitate a smooth transition of care.
• Monitor patient progress and adjust care plans as necessary to achieve optimal outcomes.

Required Qualifications

• Active Registered Nurse (RN) license.
• Minimum of three (3) years of recent experience in Acute Care Case Management.
