We are seeking an experienced Registered Nurse (RN) for a Care Manager position in Chico, California. This role is pivotal in coordinating comprehensive care and services for assigned patients throughout their episode of illness or treatment within an acute care setting, potentially including the Emergency Department.
The Care Manager utilizes advanced clinical skills and the nursing process to assess, plan, intervene, and evaluate patient care. This position involves close collaboration with patients, families, physicians, the interdisciplinary care team, and payors to ensure efficient, cost-effective care delivery and optimal clinical and resource outcomes. Key responsibilities include utilization management, quality reviews, patient advocacy, and leadership within the care team.
Key Responsibilities
Coordinate comprehensive care for assigned patients throughout their episode of illness or treatment.
Collaborate effectively with patients, families, physicians, the interdisciplinary care team, and payors.
Apply advanced clinical skills, including assessment, planning, intervention, and evaluation, to facilitate patient care.
Promote and evaluate the effective utilization of healthcare resources.
Serve as a leader within the patient care team to achieve optimal clinical and resource management outcomes.
Conduct quality and utilization reviews, analyzing admission/discharge criteria, observation/1-day stays, 30-day readmissions, and Medicare/DRG criteria.
Participate in ongoing outlier reviews.
Advocate for patients and families, facilitating communication among providers and monitoring services rendered.
Initiate appropriate clinical pathways based on admission diagnoses.
Serve as a resource person for utilization management purposes.
May include assignments within the Emergency Department.
Required Qualifications
Current Registered Nurse (RN) license.
Minimum of one (1) year of recent Case Management experience in an acute care setting.
