Registered Nurse Case Manager Job Summary

Join Tuba City Regional Health Care Corporation, a JCAHO accredited 73-bed facility located in the culturally rich Northwestern Arizona. This facility serves as a vital referral center for a 4,400 square mile area, providing comprehensive inpatient and outpatient services to the Navajo, Hopi, and Southern Paiute Reservations, alongside a diverse community of Anglo, Hispanic, and African American residents.

As a Registered Nurse Case Manager, you will play a critical role in supporting identified high-risk patients across both outpatient and inpatient settings, including specialty clinics at Tuba City Regional Health Care Corporation and Sacred Peaks Healthcare Center. Your primary focus will be to ensure quality, cost-effective healthcare by assessing essential health and social needs, facilitating seamless care transitions from admission through discharge, and coordinating post-hospital referrals.

This position involves working with a diverse patient population facing multiple barriers to self-sufficiency, such as illiteracy, welfare dependency, domestic violence, substance abuse, and mental health issues. You will serve as a key resource for nursing staff, physicians, and ancillary departments on transitional and regulatory matters, advocating for patients of all ages, from newborn to geriatric.

Key Responsibilities
  • Serve as a supportive contact for identified high-risk patients, specifically focusing on specialty clinics requiring dedicated Case Management attention.
  • Assess essential health and social needs for patients, ensuring quality and cost-effective healthcare delivery.
  • Facilitate care transitions from admission through discharge, ensuring continuity of care, alignment of services, and appropriate post-hospital referrals to continuing care agencies.
  • Act as a resource for nursing, physicians, and ancillary departments regarding transitional and regulatory issues.
  • Conduct comprehensive patient assessments, develop individualized service plans, acquire necessary resources, monitor patient progress, and proactively respond to emerging client needs.
  • Collaborate effectively with an interdisciplinary team to assess care plans, monitor patient progress, and make recommendations to ensure safe and efficient hospitalizations.
  • Evaluate patients presenting in the Emergency Department for medical necessity and care coordination requirements.
  • Assess potential admissions and work closely with advanced practice clinicians and physicians to determine appropriate admission status.
  • Propose and implement outpatient plans for safe discharge from the Emergency Department, with physician approval.
  • Coordinate care for patients with complex challenges, including abuse, chronic pain, and frequent utilization issues.
  • Interact with individuals experiencing mental health issues to help them access and navigate essential resources for improved well-being.
Required Qualifications
  • Associate’s or Bachelor’s Degree in Nursing.
  • Three (3) years of clinical nursing experience.
  • One (1) year of experience in an Outpatient/Inpatient setting.
  • Demonstrated knowledge of case management, contract health, and utilization review processes.
  • Fingerprinting must be submitted and cleared prior to start.
